Admission Forms
If you are registering a new Kindergartener, applications will be accepted January 29, 2008. Applications are due in the office by Monday, February 4, 2008 at 3:00 pm. Each family must include an application fee of $100.00. Letters of acceptance will be mailed on or about Friday, February 8, 2008. Students will be conditionally accepted into Kindergarten with full acceptance contingent on displayed readiness on the Kindergarten assessment. Applications for Grades 1-8 will be accepted starting January 28, 2008. The registration application is to be returned to the school office with the $100.00 application fee (per family). The Student Assesment Form and Record Request Form should be forwarded to the child's current school, who will then forward the records to St. Raphael School office. Once the records are received, they will be reviewed by the principal. As soon as an available opening is known, we will contact each family to inform them of acceptance. A non-refundable New Student Fee will be required within one week of acceptance to confirm the enrollment for the 2008-2009 school year.
